Getting Started Survey
To be completed any time between your initial assessment and the first week of starting your total diet replacement products
Find out how to be involved in the study as a participant or as a collaborator.
If you have been referred to the NHS Low Calorie Diet programme you are invited to take part in a short survey about your experience . Before you decide to take part or not, you’ll find information about the study on our Survey Information page, and contact details for the study team if you’d like to ask any further questions on our Contact Page.
Note: you do not need to complete the online survey all in one go — you can return to where you left off for up to 7 days as long as you use the same device, and have not finished the survey.
Please note you will need require your referral ID to start the survey. If you don’t already have your referral ID you can request this from your service provider.

To be completed any time between your initial assessment and the first week of starting your total diet replacement products
To be completed any time between the last week of your TDR phase and the first week of your food reintroduction phase.
To be completed any time between the last week of your food reintroduction phase and the first week of your maintenance phase.
To be completed any time during the your last two weeks on the maintenance phase of the programme.
To be completed ONLY if you have decided to withdraw from the programme before completing it. This survey can be completed at any point after you’ve withdrawn.
